(1) An eligible voter wishing to circulate a referendum petition shall file an application with the local clerk.
(2) The application shall contain: (a) the name and residence address of at least five sponsors of the referendum petition; (b) a certification indicating that each of the sponsors is a resident of Utah; (c) a statement indicating that each of the sponsors has voted in an election in Utah in the last three years; (d) the signature of each of the sponsors, acknowledged by a notary public; and (e) (i) if the referendum challenges an ordinance or resolution, one copy of the law; or (ii) if the referendum challenges a local law that is not an ordinance or resolution, a written description of the local law, including the result of the vote on the local law.
