34-23A-10.5. (Text of section effective January 1, 2020) Form required by 34-23A-10.1(1) to be prescribed by department. To meet the requirement of subdivision 34-23A-10.1(1), a physician shall use a form prescribed by the department. The department shall promulgate rules pursuant to chapter 1-26 specifying the form necessary to meet the requirement of this section.
