Every physician in this state attending on or called in to visit a patient whom the physician believes to be suffering from an occupational disease as defined in section 4123.68 of the Revised Code shall, within forty-eight hours from the time of making such diagnosis, send to the bureau of workers' compensation a report stating:
(A) Name, address, and occupation of patient;
(B) Name and address of business in which employed;
(C) Nature of disease;
(D) Name and address of employer of patient;
(E) Such other information as is reasonably required by the bureau. The reports shall be made on blanks to be furnished by the bureau. A physician who sends the report within the time stated to the bureau is in compliance with this section.
Reports made under this section shall not be evidence of the facts therein stated in any action arising out of a disease therein reported.
The bureau shall, within twenty-four hours after the receipt of the report, send a copy thereof to the employer of the patient named in the report.
