(a) record the revocation in the patient's medical record;
(b) cancel any orders implementing the decision to withhold or withdraw treatment; and
(c) notify the hospital staff directly responsible for the patient's care of the revocation and any cancellations. 3. Any member of the medical or nursing staff, other than a nurse practitioner, informed of a revocation made pursuant to this section shall immediately notify an attending physician or attending nurse practitioner of the revocation.
